Invoice & Infomedics
For our financial administration, we collaborate with Infomedics, a company specialized in such services. In many cases, Infomedics submits the invoice directly to your health insurer. This process is called clearing. Depending on your supplemental dental insurance, (a part of) the bill may be reimbursed.
Your invoice consists of fixed rates. You can find all rates in our price list.
The dental invoice is composed of the various services performed during a treatment. Each service on the invoice is described with a code.
Rates are determined annually by the Dutch Healthcare Authority.
You can inquire with your health insurer about which treatments they cover. You can also ask your insurer about the reimbursement amount for each service.

Did you know that dental care is free for those under 18?
It’s good to know that the basic health insurance covers dental check-ups and care until you turn 18. This means children can visit the dentist for free! Together, we can establish a healthy foundation for both the primary and permanent teeth.
Starting regular check-ups and preventive care early helps prevent dental problems in the future.
Payment in instalments is possible
Infomedics has a payment term of 30 days. If you prefer to pay in instalments or are unable to pay on time, you can set up a payment arrangement. You can easily do this through the Infomedics website. If you’d rather call, please contact the Infomedics Service Desk at 036-2031900. They will be happy to assist you.

Cancellation
If you are unable to attend an appointment, please let us know.
If you are unable to make it due to unforeseen circumstances or illness, you must cancel your appointment at least 24 hours in advance.
When you cancel your appointment in a timely manner via phone or email, we can assist someone else and you will avoid being charged for the reserved time.

Your Privacy and Personal Data at Evidentaal Dental Practice
We would like to inform you about the processing of your personal data. We store your personal and medical data digitally to provide you with the best possible care. We do this in accordance with our privacy and data security policy. By receiving care from our practice, you agree to this policy. By reading this form, you acknowledge that you are aware of your rights and agree to our processing of your data.
How We Handle Your Personal Data:
- We store your personal data and use it to provide you with quality care.
- We only share your data with third parties if necessary for delivering good care. If specific consent is required by law, we will ask for your permission in advance.
- We process your data based on the treatment agreement as described in the Medical Treatment Agreement Act (WGBO).
- You can view parts of your personal data. Please send us a written request to do so.
- If you believe that the data we have stored about you is incorrect, you may request in writing that we adjust your data.
- You can request us to delete your personal data. If we do so, we may no longer be able to provide you with responsible care. The WGBO requires a retention period of 15 years.
- Therefore, in this case, we will not delete your data but will label it as ‘inactive’.
- You can withdraw your consent for our processing of your personal data. In this case, we may no longer be able to guarantee responsible care, and we will label your data as ‘inactive’.
- You may object if you disagree with how we process your personal data.
- You will be notified if there is an issue with your personal data.
- We will not retain your data longer than necessary for providing quality care. We adhere to the retention periods set by the WGBO.
- All of the above also applies to personal data we receive from third parties.
